Yes there is -- check my response carefully and make the changes:

Your

<form action="mail.php" method="post"/> (line 73) -- you have an erroneous /

should be

<form action="mail.php" method="post">

and your

<form/> (line 83) -- you have the closing / in the wrong position

should be
